Care Coordinator

The Care Coordinator supports patients by organizing and managing their healthcare services to ensure seamless, high-quality care. This role serves as a liaison between patients, families, and healthcare providers.

Responsibilities:

  • Coordinate appointments, treatments, and follow-up care.
  • Communicate with patients, families, and clinical teams to address needs.
  • Monitor patient progress and provide education on care plans.
  • Maintain accurate patient records and ensure compliance with policies.
  • Facilitate access to community resources and support services.

Skills & Experience:

  • Experience in healthcare, case management, or patient services preferred.
  • Strong communication, organizational, and problem-solving skills.
  • Knowledge of healthcare systems and patient care processes.
