Position Overview
The Social Worker assesses residents’ mental and emotional needs, provides direct clinical services, and supervises Case Managers. This role coordinates treatment, crisis intervention, and service planning to support family stability and well-being.

Key Responsibilities

-Assess and evaluate residents referred by NYC DHS; complete comprehensive evaluations at intake

-Develop and monitor Individual Living Plans (ILPs) for families

-Provide clinical supervision to Case Managers and review case documentation

-Deliver crisis intervention and on-call support as needed

-Coordinate mental health, psychiatric care, and external referrals

-Facilitate clinical case conferences, groups, and staff meetings

-Monitor resident safety, compliance, and record quality

-Collaborate with internal teams and external service providers

-Participate in trainings, supervision, and program reporting

-Perform other duties as assigned by the Social Work Supervisor.

Qualifications

-Master’s degree in Social Work (required)

-Minimum 3 years’ experience working with homeless populations

-Knowledge of DHS regulations and CARES reporting

-Strong clinical assessment, documentation, and communication skills

-Experience working with diverse and underserved populations

-Bilingual (Mandarin or Spanish)

-Proficiency in Microsoft Office and ability to work a flexible schedule

Schedule : Monday-Friday 9AM-5PM

Location: Flushing, Queens 

Pay Rate: 35 per hour
