Case Worker

The Case Worker provides support and resources to individuals and families, helping them navigate social, health, and community services. This role ensures clients receive appropriate care and assistance to improve their well-being.

Responsibilities:

  • Assess client needs and develop individualized care or service plans.
  • Connect clients with community resources, benefits, and support services.
  • Monitor client progress and adjust plans as needed.
  • Advocate for clients and provide guidance on social, health, and legal issues.
  • Maintain accurate case records and comply with organizational policies.

Skills & Experience:

  • Bachelor’s degree in social work, psychology, or related field preferred.
  • Experience in social services, counseling, or case management.
  • Strong communication, organizational, and problem-solving skills.
