Responsibilities

  • Report to all assignments on time and in uniform, maintaining a professional and polished appearance at all times.
  • Prepare and organize care coordination stations, ensuring all necessary equipment and documentation are ready for efficient patient management.
  • Assist in managing patient flow through treatment plans and medical procedures, ensuring a smooth and organized environment for both patients and healthcare providers.
  • Communicate effectively with patients, healthcare teams, and insurance providers to ensure accurate care delivery and optimal patient outcomes.

Skills & Experience

  • Care coordination certification required (e.g., from the Case Management Society of America or the American Case Management Association).
  • At least 1 year of experience in care coordination or a healthcare setting preferred.
  • Active state certification or licensure as a care coordinator required.
  • Familiarity with patient management systems, insurance procedures, and healthcare protocols is highly valued.
