Care Manager

The Care Manager coordinates and oversees patient care to ensure effective, safe, and timely healthcare services. This role works closely with patients, families, and healthcare providers to optimize outcomes and support overall well-being.

Responsibilities:

  • Assess patient needs and develop individualized care plans.
  • Coordinate healthcare services across providers, facilities, and community resources.
  • Monitor patient progress and adjust care plans as needed.
  • Educate patients and families on treatment options and self-care.
  • Maintain accurate documentation and ensure compliance with regulations.

Skills & Experience:

  • Experience in healthcare, case management, or care coordination.
  • Strong communication, organizational, and problem-solving skills.
  • Knowledge of healthcare systems, patient care processes, and relevant regulations.
