Responsibilities

  • Report to all assignments on time and in a professional manner, maintaining a polished appearance and representing the organization with integrity.
  • Coordinate and organize client caseloads, ensuring that all necessary documentation and resources are in place for effective case management.
  • Assist in managing client appointments and service delivery, ensuring a clean, safe, and organized environment for all interactions.
  • Communicate effectively with clients, healthcare providers, and other support staff to ensure smooth coordination of services and accurate case documentation.

Skills & Experience

  • Certification in Case Management or a related field (e.g., from the Commission for Case Manager Certification or the American Case Management Association) is required.
  • At least 1 year of experience in case management or healthcare coordination preferred.
  • Active state certification or licensure in case management, social work, or a related field is required.
  • Familiarity with care coordination, client assessments, and healthcare regulations is highly valued.
