RN Case Manager

The RN Case Manager coordinates and manages patient care across healthcare settings, ensuring safe, effective, and efficient treatment plans. This role works closely with patients, families, and interdisciplinary teams to optimize outcomes and resource utilization.

Responsibilities:

  • Assess patient needs and develop individualized care plans.
  • Coordinate services across providers, facilities, and community resources.
  • Monitor patient progress and adjust care plans as needed.
  • Educate patients and families on treatment options and self-care.
  • Ensure documentation complies with regulatory standards and quality measures.

Skills & Experience:

  • Active RN license.
  • Experience in case management, care coordination, or clinical nursing.
  • Strong clinical judgment, communication, and organizational skills.
  • Knowledge of healthcare regulations, insurance, and utilization management.
