Overview:
The Supportive Housing Case Manager provides direct support to chronically homeless adults in a permanent supportive housing (PSH) program. This role helps residents achieve stability, independence, and well-being by connecting them to community resources and coordinating individualized services. The ideal candidate MUST have a background working with the chronically homeless population, including individuals with Serious Mental Illness (SMI) and co-occurring (dual) diagnoses.
